In order to notarise doctor’s reports or letters the Notary needs to get confirmation that the report/letter is genuine. This is the case even when there is an original doctor stamp on the document.
Here is the procedure:
- Scan the report/letter and email the doctor’s clinic.
- In the email please include the following in the cc to the email: pt@siriuslaw.com.au.
- The email can say:
“Attached is a scanned copy of a letter/report issued by one of the doctor’s in your medical clinic. In order for a notary to verify the document I require the medical practice to confirm a) it is genuine and b) that the attached is a scanned copy of the original.
PLEASE reply with CONFIRMED.
PLEASE reply to ALL in the cc since you need to confirm this directly to the notary. Your prompt reply is requested. Thank you for your time on this matter. Regards
