The Notary will need to obtain confirmation from the Doctor’s Surgery that the Medical report or Results are genuine. Below is an email you can send. Note: please cc me in the email as pt@siriuslaw.com.au
Dear Medical Practice [insert a name if you have one]
My name is xx and I am a patient at your Medical Practice. I obtained documentation from your Practice. I need to get the medical document notarised and then legalised at DFAT. In order to do this the Notary requires your Medical Practice to confirm the following (either the Doctor or Medical Administration staff is fine):
- The attached document/s is genuine; and
- The attached document is a true copy of the original issued.
You can simply reply with I CONFIRM. Please reply to ALL in the cc as I have the Notary included and she needs a direct reply.
